Consent and Waiver

As Body Conditioning, the provider of body conditioning services (the "Services"), we kindly request that you carefully read and agree to the following terms.

I acknowledge my choice to receive the Services from an employee or independent contractor (collectively, the “Practitioner”) associated with Body Conditioning. I have observed and understand the nature and potential intensity of the body conditioning programs offered by Body Conditioning. I am aware that, though unlikely, participation in these programs may carry risks of physical harm, including but not limited to muscular damage, skeletal damage, or nerve damage, which may necessitate medical attention and hospitalization.

I voluntarily accept full responsibility for all the risks I expose myself to while receiving the Services at Body Conditioning. By assuming these risks, I am aware that I am relinquishing any rights to potential claims I might otherwise have.

In the event of pain or discomfort during a session, I commit to promptly notifying the Practitioner so that adjustments can be made to ensure my comfort. I will not hold the Practitioner responsible for any pain or discomfort I might experience during or after the session.

I affirm that I am not aware of any medical conditions that would increase my risk of injury while receiving the Services. Furthermore, I confirm that I have not informed Body Conditioning of any reasons that would render me unfit for the Services, which could include muscular damage, skeletal damage, or nerve damage, among others.

I comprehend that the Services are not a substitute for medical care, and the Practitioner is not qualified to perform spinal or skeletal adjustments, diagnose, prescribe, or treat physical or mental illnesses.

In full consideration of the aforementioned risks and hazards, and with the understanding that I am voluntarily accepting the services provided by Body Conditioning. I willingly waive, release, and absolve Body Conditioning and its owners, board of directors, affiliates, agents, officers, employees, and independent contractors from any and all liability, claims, demands, actions, causes of action, rights of action, and/or damages of any kind linked to my receipt of the Services.

I have thoroughly read the terms and understand the assumption of risk and release of liability, as well as all the terms and conditions included herein. By booking with us, I release Body Conditioning., its owners, board of directors, affiliates, agents, officers, employees, and independent contractors from any and all claims. I acknowledge that by voluntarily agreeing to the terms, I am waiving important legal rights.
